This site occupies Suite 460 of the contemporary Jordan Commons Office Tower conveniently located just off Interstate 15 at 9350 South 150 East, Sandy, Utah.

The doctors perform surgery at Salt Lake Regional Medical Center, The Avenues Surgical Center, LDS Hospital, and Primary Children's Medical Center.
